Page 1: weekly diary grid
A week at a glance, with a box for before and after each meal plus bedtime, and a notes row each day - the classic diabetes diary layout for spotting patterns.
Page 2: everyday log
22 rows for date, time, your reading in mmol/L, when it was taken (fasting, before or after a meal, bedtime) and notes - for keeping an eye on things over weeks.
How to use it
- Wash and dry your hands before a finger-prick test - food or juice on your fingers can read falsely high.
- Fill in only the boxes your diabetes team asks you to. You do not need to complete every one.
- Note anything that affected a reading - a meal, activity, illness or a hypo - so patterns are easier to see.
- Not sure what a reading means? Use our blood sugar checker.
